SRSEQ Sexual Relationship & Experience Questionnaire

The Sexual Relationship and Sexual Experience Questionnaire (SRSEQ) is a specialist sexological assessment instrument developed by Kai at Polysoma to support trauma-informed understanding of sexual experience, consent, distress, regulation, and relational safety. SRSEQ is designed for use within psychosexual therapy, sexology and sexual and reproductive health to deepen clinical conversations about sexual wellbeing, boundaries, and relationship dynamics, particularly where experiences of sexual violence, dissociation, or complex trauma may be present.

Purpose and Clinical Focus

SRSEQ assists clinicians and clients to explore key domains of sexual experience, including:

Sexual frequency, initiation, and desire.
Consent, boundaries, and feelings of safety during sexual activity and touch.
Dissociation, numbness, and shutdown responses in the context of intimacy and sexual contact.
Sexual regulation, compulsivity, and the use of sex or self-pleasure to cope with distress, loneliness, or overwhelming emotions.
Relational power dynamics, emotional closeness, and sexual and relationship satisfaction.
Body shame, self-comfort, and patterns of sensual and sexual self-touch.

The instrument is intended to support nuanced, non-judgemental clinical dialogue about sexual wellbeing, safety, and agency, within a trauma-informed and sex-positive framework.

Research and Validation Status

SRSEQ is a specialist clinical assessment instrument developed from sexological practice, trauma‑informed counselling experience, and contemporary sexual health theory. At this stage, SRSEQ has not yet undergone formal psychometric validation studies, such as factor analysis or reliability and validity testing across large samples, and should be understood as an emerging, practice‑based tool rather than a scientifically standardised measure. SRSEQ is intended to complement, not replace, existing validated instruments, and clinicians are responsible for integrating SRSEQ findings cautiously and contextually alongside other assessment data, clinical judgment, and established evidence‑based measures.

It is designed to support and deepen clinical formulation, while also giving clients a structured, non‑judgemental way to reflect on sensitive areas of sexual experience that may otherwise be difficult to name or discuss directly. In this way, SRSEQ can help lay the foundation for further questions during intake and ongoing assessment, with positive or elevated indicators prompting careful exploration in clinical interview and, where clinically indicated, the use of additional validated psychometric instruments.

Intended Users and Eligibility

SRSEQ is a specialist tool and is not a general-purpose checklist. To safeguard client safety and quality of care, use of SRSEQ is restricted to practitioners who meet specific training and qualification standards.

SRSEQ is intended only for use by practitioners who:

Hold formal sexology qualifications, for example postgraduate training in sexology or an equivalent recognised credential in sexual health or sex therapy.
Have completed a Sexual Attitude Reassessment (SAR) course.
Work within a scope of practice that includes sexual health, psychosexual therapy, sexology and trauma-informed counselling.
Are currently a professional member of a recognised sexology or sex therapy professional body and hold active membership.

Completion of a Sexual Attitude Reassessment (SAR) course is a core requirement for competent and ethical use of SRSEQ. Practitioners without SAR training should not administer this instrument.

Professional Responsibility and Risk

Use of SRSEQ by individuals who do not meet these training and eligibility requirements may fall outside their scope of practice, may breach professional or ethical standards, and may expose them to clinical, regulatory, or legal risk. Practitioners are responsible for ensuring that their qualifications, insurance, and professional registration adequately cover the use of specialist sexological assessment instruments such as SRSEQ.
