Sex Therapy Readiness Questionnaire

Wondering if sex therapy is right for you at this point in your life?

This short questionnaire is designed to help you reflect on your readiness and clarify what you’re hoping to explore or work on in therapy. Please take your time and respond as thoughtfully and honestly as you can. Your answers will help you get a sense of where you are right now and highlight areas where professional support may be most helpful — for example around trauma, shame, desire, intimacy, ethical non‑monogamy, or other aspects of your sexual and relational world that feel important to you

Finding Your Voice

How comfortable would you feel speaking about intimate or sexual parts of your life in a professional space?
  • I feel able to be open.
  •  could be open with support.
  • I feel nervous, but willing.
  • I’m not ready for that yet.
How easy is it for you to name your needs, boundaries, desires, or concerns?
  • I can usually put words to them.
  • I can name some things.
  • I often struggle to say them aloud.
  • I tend to keep these parts hidden.

Needs, Wounds & Hope

What feels most true about what is bringing you here?
  • I want support around trauma or painful past experiences.
  • I want help with shame, fear, or disconnection.
  • I want to explore pleasure, desire, or intimacy.
  • I want to understand myself more deeply.
How clear are you about what you are hoping for?
  • I know what I want support with.
  • I have a sense, even if it’s not fully clear.
  • I’m still finding my way.
  • I just know something needs care.

Showing Up for Yourself

How ready do you feel to begin this process?

  • I feel ready to begin.
  • I feel mostly ready.
  • Part of me is ready, part of me is unsure.
  • I’m not sure if now is the right time.
How willing are you to stay curious about both the hard and hopeful parts of this work?
  • Very willing.
  • Somewhat willing.
  • Cautious, but open.
  • Not ready for that yet.

Shaping Therapy Together

How open are you to working together in a way that feels steady, respectful, and tailored to you?
  • Very open.
  • Open, though I may need time to trust.
  • Cautious, but willing.
  • Unsure at this stage.
If something feels too much, too fast, or not quite right, how likely are you to say so?
  • I would feel comfortable saying that.
  • I think I could, with support.
  • I may find that difficult, but I’d like to try.
  • I would probably keep it to myself.

Overall Readiness

Remember: there is no perfect readiness score. These reflections are simply here to help you get a feel for what sex therapy may involve, and whether this feels like the right kind of support for you right now.

If you feel ready to begin — whether you are seeking healing, understanding, pleasure, or a deeper connection with yourself — I would be honoured to hear from you.
